Hui Zhu 67a06fdfbf mm: fix ASSERT_EXCLUSIVE_BITS by passing memdesc_flags_t by pointer
KCSAN reports a data race between page_to_nid()/folio_pgdat() reading
page->flags and folio_trylock()/folio_lock() concurrently doing
test_and_set_bit_lock(PG_locked, ...) on the same word, e.g.:

  BUG: KCSAN: data-race in __lruvec_stat_mod_folio / shmem_get_folio_gfp

The race is benign: nid/zone bits are set once at page init and never
overlap with PG_locked. However, ASSERT_EXCLUSIVE_BITS() inside
memdesc_nid/zonenum() was checking a by-value copy of the flags word, not
the live page->flags, so it failed to annotate the real access.

Change memdesc_nid(), memdesc_zonenum(), memdesc_section(), and
memdesc_is_zone_device() to take a const memdesc_flags_t * and update all
callers to pass &page->flags / &folio->flags, so ASSERT_EXCLUSIVE_BITS()
operates on the actual shared word.

Guard the ASSERT_EXCLUSIVE_BITS() call in memdesc_zonenum() under
ZONES_WIDTH != 0 to avoid a zero-mask check on configs where the zone
field is absent. memdesc_section() needs no such guard, since
SECTIONS_WIDTH is never 0 wherever SECTION_IN_PAGE_FLAGS is defined.
Under CONFIG_NUMA=n, memdesc_nid() itself is stubbed to "return 0" instead
of reading page->flags, since NODES_MASK is 0 and the check can never
fire; page_to_nid()/folio_nid() now just call memdesc_nid()
unconditionally and rely on that stub, instead of duplicating the
CONFIG_NUMA split at each call site.

Thomas Huth 5339ec29f9 mm: replace __ASSEMBLY__ with __ASSEMBLER__ in memory management header files
While the GCC and Clang compilers already define __ASSEMBLER__
automatically when compiling assembly code, __ASSEMBLY__ is a macro that
only gets defined by the Makefiles in the kernel. This can be very
confusing when switching between userspace and kernelspace coding, or when
dealing with uapi headers that rather should use __ASSEMBLER__ instead.
So let's standardize now on the __ASSEMBLER__ macro that is provided by
the compilers.

This is a completely mechanical patch (done with a simple "sed -i"
statement).

Mike Rapoport (Microsoft) 8268af309d arch, mm: set max_mapnr when allocating memory map for FLATMEM
max_mapnr is essentially the size of the memory map for systems that use
FLATMEM. There is no reason to calculate it in each and every architecture
when it's anyway calculated in alloc_node_mem_map().

Drop setting of max_mapnr from architecture code and set it once in
alloc_node_mem_map().

While on it, move definition of mem_map and max_mapnr to mm/mm_init.c so
there won't be two copies for MMU and !MMU variants.

Christoph Hellwig 3e25d5a49f asm-generic: add an optional pfn_valid check to page_to_phys
page_to_pfn is usually implemented by pointer arithmetics on the memory
map, which means that bogus input can lead to even more bogus output.

Powerpc had a pfn_valid check on the intermediate pfn in the page_to_phys
implementation when CONFIG_DEBUG_VIRTUAL is defined, which seems
generally useful, so add that to the generic version.

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
   COPYING file license applied.

For non */uapi/* files that summary was:

   SPDX license identifier # files
   ---------------------------------------------------|-------
   GPL-2.0 11139

and resulted in the first patch in this series.

If that file was a */uapi/* path one, it was "GPL-2.0 WITH
Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:

   SPDX license identifier # files
   ---------------------------------------------------|-------
   GPL-2.0 WITH Linux-syscall-note 930

and resulted in the second patch in this series.

 - if a file had some form of licensing information in it, and was one
of the */uapi/* ones, it was denoted with the Linux-syscall-note if
any GPL family license was found in the file or had no licensing in
it (per prior point). Results summary:

   SPDX license identifier # files
   ---------------------------------------------------|------
   GPL-2.0 WITH Linux-syscall-note 270
   GPL-2.0+ WITH Linux-syscall-note 169
   ((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21
   ((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17
   LGPL-2.1+ WITH Linux-syscall-note 15
   GPL-1.0+ WITH Linux-syscall-note 14
   ((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5
   LGPL-2.0+ WITH Linux-syscall-note 4
   LGPL-2.1 WITH Linux-syscall-note 3
   ((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3
   ((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1

and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

mainline in the form of 012dcef3f0 "mm: move __phys_to_pfn and

Christoph Lameter 8f6aac419b Generic Virtual Memmap support for SPARSEMEM
SPARSEMEM is a pretty nice framework that unifies quite a bit of code over all
the arches. It would be great if it could be the default so that we can get
rid of various forms of DISCONTIG and other variations on memory maps. So far
what has hindered this are the additional lookups that SPARSEMEM introduces
for virt_to_page and page_address. This goes so far that the code to do this
has to be kept in a separate function and cannot be used inline.

This patch introduces a virtual memmap mode for SPARSEMEM, in which the memmap
is mapped into a virtually contigious area, only the active sections are
physically backed. This allows virt_to_page page_address and cohorts become
simple shift/add operations. No page flag fields, no table lookups, nothing
involving memory is required.

The two key operations pfn_to_page and page_to_page become:

   #define __pfn_to_page(pfn)      (vmemmap + (pfn))
   #define __page_to_pfn(page)     ((page) - vmemmap)

By having a virtual mapping for the memmap we allow simple access without
wasting physical memory. As kernel memory is typically already mapped 1:1
this introduces no additional overhead. The virtual mapping must be big
enough to allow a struct page to be allocated and mapped for all valid
physical pages. This vill make a virtual memmap difficult to use on 32 bit
platforms that support 36 address bits.

However, if there is enough virtual space available and the arch already maps
its 1-1 kernel space using TLBs (f.e. true of IA64 and x86_64) then this
technique makes SPARSEMEM lookups even more efficient than CONFIG_FLATMEM.
FLATMEM needs to read the contents of the mem_map variable to get the start of
the memmap and then add the offset to the required entry. vmemmap is a
constant to which we can simply add the offset.

This patch has the potential to allow us to make SPARSMEM the default (and
even the only) option for most systems. It should be optimal on UP, SMP and
NUMA on most platforms. Then we may even be able to remove the other memory
models: FLATMEM, DISCONTIG etc.

Randy Dunlap f05b6284ee [PATCH] typeof __page_to_pfn with SPARSEMEM=y
With CONFIG_SPARSEMEM=y:

mm/rmap.c:579: warning: format '%lx' expects type 'long unsigned int', but argument 2 has type 'int'

Andy Whitcroft 67de648211 [PATCH] squash duplicate page_to_pfn and pfn_to_page
We have architectures where the size of page_to_pfn and pfn_to_page are
significant enough to overall image size that they wish to push them out of
line. However, in the process we have grown a second copy of the
implementation of each of these routines for each memory model. Share the
implmentation exposing it either inline or out-of-line as required.

KAMEZAWA Hiroyuki a0140c1d85 [PATCH] remove zone_mem_map
This patch removes zone_mem_map.

pfn_to_page uses pgdat, page_to_pfn uses zone. page_to_pfn can use pgdat
instead of zone, which is only one user of zone_mem_map. By modifing it,
we can remove zone_mem_map.

KAMEZAWA Hiroyuki a117e66ed4 [PATCH] unify pfn_to_page: generic functions
There are 3 memory models, FLATMEM, DISCONTIGMEM, SPARSEMEM.
Each arch has its own page_to_pfn(), pfn_to_page() for each models.
But most of them can use the same arithmetic.

This patch adds asm-generic/memory_model.h, which includes generic
page_to_pfn(), pfn_to_page() definitions for each memory model.

When CONFIG_OUT_OF_LINE_PFN_TO_PAGE=y, out-of-line functions are
used instead of macro. This is enabled by some archs and reduces
text size.
